Surgery time - 25 minutes

Placed in dorsal recumbency.  Standard sterile prep of R tarsometatarsal
area and L foot (D4).  Incised over palpable pellet at medial
R TMT and used mosquito hemostats to remove the pellet that was
partially embedded in the bone.  Flushed with sterile saline.
 Closed skin with 5-0 PDS SI (2 sutures).  Applied TAO and tegaderm
over incision to protect from contamination.  Incised at medial
aspect of proximal L D4.  Was able to remove some small fragments
of lead pellet, but the intact pellet seen radiographically was
embedded in the bone and could not be removed without refracturing
it.  Flushed the site and closed the skin with 5-0 PDS. Applied
TAo and Telfa. Placed an interdigital bandage on the L foot.

Found scattered mineral density material in GIT consistent with small stones or pieces of bone.  There are multiple metal projectiles present - distal tip of L wing, caudoventral L coelom, R medial mid-tarsometatarsus associated with chronic fracture, L D4 also associated with chronic fractures.  The projectile injuries are not acute, at least 6w old, but could be much older.  There is boney remodeling around the TMT fracture, less remodeling seen at the digit.

Plan: Pending AI results, blood should be drawn to run a CBC, Chemistry panel and electrophoresis. Radiographs should also be taken to assess the left shoulder to determine cause of instability.
